Continuous Care

Ameriprime Hospice remains committed to assisting individuals during challenging times. When continuous medical attention is required, we coordinate the transfer to a nursing facility where around-the-clock supervision can be provided. This is often necessary when additional nursing oversight is needed for symptom control or medical management. Even after the transfer, our team continues to communicate with family members, the primary physician, and the hospice Interdisciplinary Team (IDT) to ensure ongoing coordination of care. Our goal is to facilitate a smooth transition while maintaining essential medical oversight.

Continuous care is arranged as needed to address symptoms and provide necessary interventions.
